LIST OF DOCUMENTS REQUIRED:

At the bottom of this page, you can find the application form necessary for the evaluation procedure. Please, fill in all details on a computer. After printing the document out, do not forget to SIGN it. If you do not submit the form filled on PC, your application will not be accepted. Make sure you are filling ONLY programmes you are accepted to with the correct application numbers and the precise name of the programmes. - Copy of your passport.
- High school leaving diploma/certificate - an official copy (verified by a notary).
Students from the Ukraine: If you did not attend leaving exam (were exempt from the final examination), you must provide a verified copy of the ZNO/NMT! - Transcript/official grade report - an official copy (verified by a notary).
The transcript must be from the last 2 years of study before the leaving exam and must include list of subjects with marks. - Confirmation of payment foreign education evaluation fee.
- Statement from your high school: the statement must state that you received your certificate/diploma after completing your secondary education studies according to the legal regulations in your country. It also must explicitly state that this education authorizes the student to enter university at Bachelor level in your country or abroad. The statement must contain that the school is accredited for providing secondary education (the name of the authority who provides the accrediation is compulsory). Moreover the statement MUST contain full name of the high school, contact e-mail, name and signature of the person who issued the statement and the school´s official stamp. If the school is a private institution additional accrediation might be requested.
Both High School Certificate and Transcript of Records MUST have a verification of authenticity. The verification procedure depends on the country where the documents were issued.
Check the section "verification of authenticity", find country you studied in and check what form of verification is required from you. Please, follow these instructions very carefully!
The documents must be either in English or Czech. Otherwise we require an official translation (from an approved sworn translator).
Other translations (e.g.: translations by any agency, plain translations or uncertified translators) will NOT be accepted.
The sheets with translations must be firmly attached to the AUTHENTICATED (notary verified copies) of the required documents to avoid any changes or replacement of the lists. Translations attached to unauthenticated copies of documents WILL NOT be accepted.
1. Make a proper verification of authenticity according to the country the documents were issued = > 2. Make a verified copy of your original documents => 3. IF necessary - Make an official translation of this verified copy (make sure the documents are firmly attached to each other).
If you change your name, you need to provide proof of the name change (usually notary verified copy of marriage certificate applies).
If some documents do not contain your full name, we need the notarized statutory declaration/affidavit, to prove the document belongs to you personally.
WE STRONGLY ADVISE TO LOOK AT THE SECTION "MOST COMMON MISTAKES" TO AVOID ANY INCONVENIENCE
Next steps:
- Once you think your documents are complete according to all requirements above (apostilles/superlegalization/correct translation, etc.) make scans and send ALL the documents in a correct order (application form - passport - diploma - transcript, …) in ONE .pdf file via e-mail first!!!
- The documents will be briefly checked from our side via e-mail. AFTER that you will get further instructions how to deliver the documents physically. Please, wait for this reply from our side!!! (due to the big amount of e-mails especially during the summer season our reply time can take a while, please be patient).
- You must pay the fee - See the section: “Payment for IE” for specific instructions.
The payment must be settled by the time your documents arrive, otherwise they will NOT be accepted!!!
Obtained documents that fulfils all criteria will be proceed to the Committee and approved/not approved by them (keep in mind we have not authority in this process as we are only the submission office). The process can take a very long time.
You are ALWAYS informed about the updates, so you do not need to send us inquiries about the process as we do not have any insight to it!
NOTE: The internal evaluation serves only the purpose of our university CZU. Therefore, you will not get any copy or certificate as it is for the internal use only. The documents MUST stay at the study department. The Internal Evaluation is valid only for one application at the certain year. You cannot reuse it and if you fail your studies, you need to apply again and do the process of Internal Evaluation from the scratch!
The documents we receive MUST stay at the CZU for the future reference and WILL NOT be returned. Hence ONLY verified copies are accepted. According to the regulations and the note above we WILL NOT accept original documents!!!
